- fill out a change of contact details form
- phone or visit the surgery from 8.30am to 5.30pm

Change of address
If you have moved house, please contact us as soon as posisble on 01465 831302 to let us know your new address. To remain registered at this practice, your new address must still be within our catchment area.

If your new address is outwith this area you will need to register with an alternative practice.

Changing your phone number or email address
Please contact us on 01465 831302 to advise us of any changes to your phone number or email address.
Changing your name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate or deed poll.
